Male breast Reduction Case – 17 Old Boy Bilateral Breast Enlargement


It is a condition in which the male breasts become enlarged and appear like a woman’s chest. It causes embarrassment and low self-esteem in men. The reason could be hormonal imbalance or some underlying medical condition. It could be an indication of an impaired liver function or a sign of cancer. Certain medications that contain estrogen might also cause the condition. People who take drugs like marijuana and anabolic steroids are also prone to develop this condition. Nearly 40 % of men undergo this condition at some point in their lives. It may happen at any age. Sometimes, it heals itself. However, it may not heal in many cases. Such cases call for a surgical treatment method.

Case report


The patient was a 17-year-old boy. He had an 18-months history of bilateral breast enlargement (breast enlargement on both sides). He also experienced tenderness in the breast mass which led to inconvenience. There were no such cases in his family history. He had not used any drugs or dermal applications. The overall health of the boy was good. As the patient’s height was 178 cm and weight 68 kg, he was not considered obese.

Upon testing, the right breast showed a  mass of 5 cm in diameter, with tenderness. It was causing a pronounced asymmetry of the right breast. There was a minor mass in the left breast which measured 3 cm.


Gynecomastia may occur in various age groups. The first age group when it is bound to occur is the neonatal period, which is seen in 60% to 90% of all new-borns. It happens due to the transfer of estrogens to the baby from the mother’s placenta. The second age group when the condition is seen commonly during puberty, due to an imbalance between estrogens and androgens. The third age group when it can be seen commonly in the age group of 50- to 80-year-old males.


The surgical procedure for male breast reduction is a two-hour process, which is carried out under general or local anesthesia. It involves the excision of the glandular tissue if it is excessive. The treatment may include only surgery or surgery with lipoplasty. Lipoplasty is a procedure in which a suction device is inserted through incisions to suck out fat.  Only liposuction may be used if there is a need for the removal of excess fatty tissue only.
